HIPAA Privacy Officer & Security Official

Conceptual Healthcare Corporation has a designated Privacy Officer (per 45 CFR §164.530(a)(1)) and Security Official (per 45 CFR §164.308(a)(2)) responsible for our privacy and security policies and for handling your questions, requests, and concerns. The role is currently held on an interim basis; dedicated staff will be added as the organization grows.

Your rights

Under HIPAA, with respect to your protected health information you may:

  • request access to and a copy of your records;
  • request an amendment of information you believe is incorrect;
  • request an accounting of disclosures;
  • request restrictions on certain uses and disclosures;
  • request confidential communications; and
  • be notified if a breach affects your unsecured information.

How to file a complaint

You may file a complaint with us at privacy@conceptualhealth.com. You also have the right to file a complaint with the U.S. Department of Health & Human Services, Office for Civil Rights (OCR) — see hhs.gov/hipaa/filing-a-complaint. You will not be penalized for filing.
