We’ll set up your patient account in a few seconds. Your personal email stays private — we generate a system login for you so a stranger can’t guess it.

How sign-in works. You enter your real email below. We assign you a login of the form <your-member#>.patient@conceptualhealth.com. That assigned address is the only one the sign-in form accepts — phishers can’t guess it from your public address. Your personal email is kept for password and two-factor recovery only.

These are policy gates — a patient account cannot hold protected health information or trade tokens without each acknowledgement. Each acceptance is timestamped, IP-stamped, and HMAC-chained on our audit ledger.

I have read and acknowledge the HIPAA Notice of Privacy Practices (45 CFR 164.520) describing how Conceptual Health® may use and disclose my protected health information. I consent to telehealth services delivered through the Conceptual Health® network, including video visits and remote prescribing where my licensed clinician determines it is appropriate. I consent to receive account-security, appointment, and care-coordination communications by email and (where a phone is on file) text message. I can adjust which channels reach me from account settings — but security alerts are required.

Other things you can defer until later: insurance, primary care provider, emergency contact, lab preferences. These are prompted from your patient hub when you first need them — but the four consents above are required at signup.
