Five-seat committee of the Trust Council. Two academic seats, two clinician seats, one patient-advocate seat. Three-of-five quorum required to enter a fraud finding. Findings are themselves chain transactions and are reviewable on appeal.

How a fraud case actually moves

Anatomy of an audit determination.

Day 0 · Signal

Anomaly detector flags clinician X-742

Mint volume on the ER axis is 4.2σ above peer-group baseline for the same specialty and panel size. Detector raises an INVESTIGATE flag. No mints are paused at this stage.

Day 1–3 · Triage

Audit-committee staff pulls a sample

Staff pulls 30 of clinician X-742's last 90 mints, blinded to identity, and sends to two independent specialist reviewers. Reviewers attest whether the documentation supports the lift on each axis.

Day 8 · Determination

14 of 30 reviewed mints are unsupported

Committee meets, reviews findings. Vote 4–1 to enter a fraud finding. Determination is signed and pinned to the chain.

Day 8 · Burn

14 reversed mints become burn entries

Compensating burn transactions zero the 14 affected wallets. The original mint events remain on the ledger; the wallets net to zero on those events. Affected patients are notified by their guardian-orb inbox with full case detail and an appeal path.

Day 14 · Public

Determination is published in plain language

Redacted case summary appears here on this page and in the public chain explorer. Identifies are anonymised; mechanism is fully described.

Day 14–60 · Appeal window

Clinician and affected patients may appeal

Appeal goes to a different five-seat panel. If the appeal succeeds, the burn is itself burned (the chain shows both the burn and the burn-of-burn) and the wallet is restored.
